A thorough understanding of the mainstream services and NDIS providers in South Australia ensures Adelaide Therapy Hub’s Support Coordinators can support you to access supports, including mainstream, informal, community supports that match your NDIS and personal goals.
We work alongside you, and with consent your support network, to assist you to explore your options and develop confidence in coordinating supports.
Once services are established, your support coordinator can continue to provide support by overseeing services to ensure they are cost- effective, meeting your needs, and progressing towards your goals.
There are three levels of Support Coordination
Support Connection which is short-term assistance to implement your NDIS plan, including identifying service providers and negotiating Service Agreements
Support Coordination which is longer-term support to build your capacity to navigate the complexity of services and supports you need from a range of providers
Specialist Support Coordination which is specialist support coordination for if you are having to manage a more complex situation.

Our Support coordinators work creatively and resourcefully with you to assist in many areas, including but not limited to:
Helping you to understand your plan, funding and options
Negotiating and scheduling support referrals to ensure you receive the correct services
Coordinating and overseeing multiple services
Researching, assessing, and engaging mainstream, community, informal and provider support
Obtaining quotes for services
Negotiating with and coordinating service providers to activate your plan
Liaising with government, NDIA, and other relevant parties on your behalf
Monitoring the quality of services
Resolving points of crisis
Facilitating information sharing between providers and stakeholders
